I've seen varied forms but I'm not sure about what the proper punctuation is, if at all.

Should each new line begin with a capital, even if it isn't the start of a new sentence?

Assuming a question mark or exclaimation mark isn't called for, is a period compulsory after the last word?

Anything else?
